Pressing the green palm buttons on both sides of the punch press initiates a single press cycle. Releasing either button should stop the cycle. Misaligned linkages and clogged air-cylinder exhaust caused the press to malfunction and crush the operator’s fingers. OSHA inspectors also found the pushbuttons too close to the press platens.
